New Clients

All potential new clients (including those coming from referrals) need to fill out our intake/screen below to be considered for an evaluation.

The buttons below will redirect you to a Google Form. You can NOT save and exit the form once you begin. The entire form should take 30-45 minutes to complete. Once this is completed, please add information@anxietyannarbor.com and frontdesk@anxietyannarbor.com to your email contacts so our follow up emails do not go to your spam.

Once the intake and screen are complete;

A staff member will follow up within a week to set up a patient portal and complete paperwork
You will receive a confirmation email that you are on our waitlist once all of your paperwork is complete.
Current wait time for an evaluation is approximately 1-3 months. Our office manager will reach out to you to schedule for your evaluation via email. (The initial evaluation would only allow us to determine if our center is the best fit for your needs or if you would be better suited seeing another provider. It is not a guarantee of ongoing treatment. However, if the clinician determines that we are an appropriate match for your concerns, we will do the best we can to get you established quickly for regular appointments. Therefore, please be advised that there may be a delay to the actual start of treatment and that treatment may be with a clinician other than the one that completed the evaluation.)
After your evaluation is complete (1-3 sessions), you will be provided with recommendations and added to our treatment list, if our clinic is a good fit for you. Your recommendations may include attending one of our classes or a medication evaluation with a psychiatrist to better prepare you for therapy. We encourage you to follow any recommendations given to get the most benefit out of your care and help shorten your time in treatment. We will contact you as soon as possible to schedule with a specialist that fits your needs based on your evaluation.
