Returning Clients
Returning clients who have been seen within the past 6 months can message their provider about returning for care through the patient portal. If you have been seen in the past 6 months and are returning for care, but would like to be seen by a new provider, please email frontdesk@anxietyannarbor.com
Past clients referred out for other therapies might be required to send records to the Anxiety and OCD Treatment Center of Ann Arbor to be reviewed before they can start treatment here.
Returning clients who have NOT been seen within the past 6 months need to complete the "Returning Client screen listed below. If you have not been seen in over 6 months, you might need to complete a new evaluation.
