Important Note about Phone Messages We are a small clinic and do not have a full-time receptionist. Phone calls will generally go to voicemail. We prefer general questions to be directed to email@example.com as opposed to phone messages whenever possible. Email messages are checked much more frequently than voicemail messages. If you have a specific question for your clinician, please contact your clinician directly.

Evaluation Appointment If you are unable to keep your scheduled Evaluation Appointment, please contact the Office Manager (email@example.com) at least 72 hours in advance of your appointment. If you do not cancel with adequate notice, you may be charged per the terms of the Evaluation Agreement. If you no-show or cancel more than once, you may also lose your place on the Evaluation Wait List (i.e., be placed at the bottom of the wait list).
Treatment Appointment If you are unable to keep your Treatment Appointment, please contact your clinician via email to cancel at least 24 hours in advance. You will be charged the full fee for sessions that are missed/ cancelled without adequate notice.
Do you have suggestions or feedback for our clinic? Please contact us by using the form to the right. If you have a concern or complaint, we encourage you to attempt to resolve it via discussion with your Clinician (if applicable), our Office Manager, or our ownership team. However, you may also submit this form regarding your concern and a clinic representative will be back in touch with you within 3 business days. ***This form is for current or prospective clients only.
